To set up an account with us, please complete the account information form (included in the package) and email or fax it with a copy of your seller’s permit or business license to us.  If you are in California, please include a completed California Resale Certificate.   If you are ready to place your first order, you can attach the order to the account information form.  You may want to use our order forms for preparing your orders.  These order forms are regularly updated and can be downloaded from our site.
